First Choice Health Network
There are many local plans that fall under the First Choice umbrella. Please call the phone number or follow the link listed on the back of your insurance card.
Medicare
After your deductible has been met for the year, the plan will cover 80% of the medical portion of your exam. The refractive portion and glasses are not covered benefits. Your supplemental plan may cover the 20% coinsurance.
We do not participate in the “After Cataract Glasses” program. Ask about our alternative packages.

Insurance companies won’t guarantee payment based on quotes given over the phone or on-line. Final determination is made when they process the claim. They may ultimately pay differently than their initial quote.
