Forms and Resources
New patients can start their registration at home. We try to keep the paperwork to a minimum. You're welcome to download copies of our intake forms and HIPAA documents on-line, fill them out at home, then just bring them to your appointment.
Returning patients only need to review and update their information at their appointment.
Getting a head start
In addition to filling out our forms, please bring the following to your appointment:
- Photo identification
- Insurance cards
- Current glasses
- Foil tops or boxes of current contact lenses
- Prescription sunglasses
- Computer glasses
