We recommend filling out the rebate form and mailing in the required information the day you pick-up your contacts.
Details and restrictions
- Rebates are only valid for contact lenses purchased at our office. Not valid at warehouses or big box stores.
- Some rebates have time limits related to the exam and/or purchase dates. Pay attention to the must be post-marked-by-date.
- Read the directions and terms & conditions on the rebate form carefully.
- Print clearly. Some rebates cannot be processed if your handwriting isn’t legible.
- Most rebates paid in the form of a Visa Prepaid Card.
- Make sure you send in all requested information. This may include original rebate form, UPC codes, exam, fitting and contact lens box receipts.
- Rebate amounts may be decreased depending on how any insurance benefits were applied. In most cases, you will not be rebated more than you paid out-of-pocket.
- Save your paperwork to help track your rebate.
- Some companies will not send rebates to PO boxes.
- Village Eye Care only supplies rebate information and does not guarantee payment. We have no control over program rules or decisions regarding payment made by the lens vendor.
